Brand is Scandia, do not know the model name, I think that tag is missing.

Fits king or queen.
Picture is of the queen fitment, king wouldn't hang down as far.

Comes from a smoke free/pet free home.
Used very little, has been stored in a giant ziploc bag when not used, bag comes with purchase, it's one of those vacuum things.
