*About the Position:*
Registered Nurse are responsible for comprehensive assessments in the patient's home, including the completion of OASIS using electronic documentation (Axxess).
* Evaluate patient and home health environment to determine required services and involve the patient and patient's family in developing the plan of care.
* Develop the POC based on findings from evaluation visit.
* Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
* Administer medications and treatments as prescribed by physician
* Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
* Participate in regular case conference and in-services
*Requirements*
* One year of previous adult home health experience required
* Current Texas Registered Nurse (RN) license
* One year of clinical nursing experience
* Current Texas driver's license
* CPR Required
* Experience with healthcare software, Axxess is a plus
* One year of previous OASIS experience
