Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our clinical team as a *Hospice On Call Registered Nurse.
* 7 on 7 off.

*Responsibilities of this position include the following:*
* Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME
* Develops, coordinates, and follows the patient's individualized Plan of Care
* Coordinate with team members/physicians and participate in staff and Interdisciplinary Team meetings
* Appropriately identify and utilize resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc.
* Serve as a patient and family advocate to ensure delivery of the highest quality of care, providing education to patients, families and caregivers
* Complete documentation in a timely manner
* Communicate effectively with coworkers and patients/family
* Participate in bereavement support
Job Type: Full-time
Pay: From $85,000.
